The Avant-garde Health CMS TEAMS Program will kick off with a webinar for collaborative members on Thursday May 8th, at 1pm and will involve both administrators and clinicians. The program will be focused on success strategies that can be utilized across all the TEAM episode areas, but there will also be content specific to clinical episodes.

Collaborative Leaders
Derek Haas is the CEO and founder of Avant-garde Health, which provides health systems, ASCs, and surgeons with comprehensive insight into their surgical care and empowers them to improve their profitability and care quality.
Avant-garde has been helping health systems succeed with bundled payments since 2016. The company's work has been featured on the front page of The Wall Street Journal. Derek has co-authored twelve Harvard Business Review articles, including How One Hospital Has Succeeded in a World of Bundled Payments and Getting Bundled Payments Right in Health Care. He has also co-authored a JAMA article evaluating the impact of the Medicare joint replacement bundled payment program. He holds a BA and MBA from Harvard.
Ann Conrath has worked in many capacities within the healthcare industry, primarily supporting organizations transition to value-based care with operational best practices and innovative technology.
Prior to joining Avant-garde Health, she developed successful go to market strategies for new technologies and created the structure and implementation of several value-based care contracts and initiatives, including maternity care home models and national bundled payment programs. Ann is Director of Sales at Avant-garde Health and has a BA from Marquette University and received her Master of Social Work degree from Washington University in St. Louis.
Dr. Porter Jones is the Vice President of Clinical Transformation at Avant-garde Health, with a strong focus on value-based healthcare (VBHC). His expertise in VBHC began at Hospital Albert Einstein in São Paulo, Brazil, where he implemented value-based care strategies, concentrating on case costing, outcome measurement, and developing Centers of Excellence in neurosurgery, orthopedics, and cardiothoracic surgery.
At Avant-garde Health, Porter oversees the research team, publishing extensively on VBHC and working closely with clients to implement best practices in value-based care. His leadership helps healthcare organizations optimize both patient outcomes and financial performance through advanced analytics and care models. Porter holds a medical degree from the University of Utah School of Medicine and an MBA from Harvard Business School.
